Enrollment Request Form


You can use this form to request enrollment, or to request additional information before enrollment.

Note that in order to register for a course you must be a VHCC student.  If you have not submitted a VHCC application you will need to do so.  Instructions are at the VHCC homepage http://www.vhcc.edu/ > Admissions

If you are not a VHCC student and have not submitted a VHCC application, please avoid possible confusion in the enrollment process by submitting your application before submitting this form.

Your email address. If you have a VCCS email address you should use it:

Your VHCC EMPLID# (if you do not have your EMPLID# you will need to go to the VHCC homepage and follow instructions for registration; if you find that you do not yet have an EMPLID#, then be sure you have completed the VHCC admissions process). 

This is not required if you are simply requesting information.

What mathematics courses have you previously completed, and when and where did you complete them?  Also list physics courses if you are interested in taking a physics course. 

Have you taken the Compass placement exam?  If so, when and where, what were your mathematics placement scores and for what courses were you recommended? 

You may add optional additional information, comments and/or questions in the box below.

IMPORTANT:  You should email the instructor (dsmith@vhcc.edu) informing him that you have completed this form, and include a copy of the Form Confirmation that will appear after you click on Submit Form below.

You should receive a reply from the instructor, confirming receipt of your information and informing you of what action has been taken, within 2 days of submitting this form (between semesters and during breaks there might or might not be an additional delay). 

If you do not receive a reply within 2 days, you should email the instructor every 2 days until you do receive a reply. 

Emails from non-VCCS email addresses are sometimes filtered out by our SPAM filter, so if you have not received a reply within a week, it probably means that your request got overlooked.  This happens only rarely, but if it does please take a minute and fill out this form again.
