Student Forms
Students Entering NUR 111, 115 or 193 or Readmitted to the VATNP Nursing Program
Proof of all the health and professional requirements must
be on file in the VATNP office at Virginia Highlands Community College prior
to participation in any clinical activity. These records are required by our
clinical agencies with which the VATNP has binding contracts. YOU WILL NOT
BE ALLOWED INTO THE CLINICAL SITES WITHOUT THIS DOCUMENTATION. Student files
will be reviewed each semester for currency of the documents. Failure to
provide this documentation by the deadline dates will result in the student
being unable to attend clinicals and therefore failing the course.
Deadlines: All paperwork must be submitted to VATNP, PO Box
828, Abingdon, VA 24212 before May 15th for bridge (LPN-RN and Paramedic-RN)
students and before August 1st for new students. (Readmitted students must
submit documentation before the first day of class upon reentering the
program.)
Do not forget to make copies of ALL documents submitted to the
VATNP. You will need copies for your own records.

2nd Year Students
