If you intend to take your tests at the VHCC learning lab, you do not need to
fill out this form.
If you will be taking your tests at a different testing center you will need to contact
that testing center and obtain an email address
so your instructor can send them a copy of testing procedures and verify that
the testing situation is acceptable to all parties.
Proctoring is generally
approved for testing centers and/or libraries of accredited institutions of
higher education, and generally in situations where testing is under the control
of faculty of such institutions.
Due to the variability
of situations at thousands of primary and secondary schools and public
libraries, proctoring is not ordinarily approved at those institutions.
Exceptions are sometimes made when the specific situation meets stringent
conditions, but even in the case of exceptions the final exam must be taken in
conjunction with an approved institution of higher education.
All proctor information must be submitted using this form.
- After submitting this form you should email the instructor to alert him to
the fact that you have submitted the form. A brief message is fine; e.g.,
'proctor information has been submitted'.
- You should also submit a copy of the same note using the
Submit Work Form. Email can be
unreliable and is difficult to search, whereas form results are very
reliable and easily searched.
You should expect a reply to your email within 48 hours, informing you of the
status of your request. If you do not get a reply, you should repeat your
email to the instructor.
Your course (e.g., Mth 151, Mth 173, Phy 121, Phy 232, etc. ):
Enter your access code in the boxes below.
Remember that it is crucial to enter your access code correctly. As
instructed, you need to copy the access code from another document rather than
typing it.
Your Name:
Your VCCS email address. This is the address you were instructed in
Step 1 to obtain. If you were not able to obtain that address, indicate
this below.
You may enter any message or comment you wish in the box below:
The message I will send your proctor assumes that you have contacted your
proctor or testing center before submitting this form. If you have not
contacted your proctor they might not know who you are, which can lead to
confusion. It is important that we conduct the process in a way that
doesn't create extra work or confusion for test proctors.
Please verify in the box below that you have contacted your proctor or
testing center:
The location of the proposed testing site (the institution and the facility
within the institution, e.g., VHCC Learning Lab):
The name (and title, if applicable) of the contact person:
Testing information will be sent to your proctor or to the testing center via
email, and an email address is therefore necessary.
Some testing centers prefer that I send information to a central address so
that all personnel at the center can access the information. You should
determine whether this is the case for your proctoring site, and if so you
should provide that email address either in addition to or instead of the
address of your specific proctor.
Please give the appropriate email address(es) in the box below.
Once more, after submitting this form, remember to take the following two
additional steps:
- After submitting this form you should email the instructor to alert him to
the fact that you have submitted the form. A brief message is fine; e.g.,
'proctor information has been submitted'.
- You should also submit a copy of the same note using the
Submit Work Form. Email can be
unreliable and is difficult to search, whereas form results are very
reliable and easily searched.
