This form is for submitting text files to your instructor.  Only the text in your file will be transmitted by the form.

As with all files you should save a backup on your hard drive or memory stick.

Title you want to see on your posted work for this assignment.  (do not use punctuation except for dashes - and underline symbols _ .  Do not use symbols like @  $  &  *  ! or commas, periods, exclamations marks, slashes or backslashes, = signs, + sign, etc.)

NOTE:  You have been instructed to include access:  name: and title: lines in emails.  This does not apply to work submitted using forms; that information has already been entered above.  Do not include lines in your submitted document that start with 'access: ', 'name:' or 'title:'.

You might or might not be submitting SEND files, depending on your course.

If you are sending SEND files you should 

1. Copy the file to a location outside your c:\vhmthphy folder.
2. Open the copy.
3. Copy the contents of the file to your clipboard without making any changes to that file (this can be done in any of several ways.  For example if you use ctrl-A you will highlight all the work in the file, then ctrl-C will copy it to the clipboard).
4. Paste everything into the box provided below (right-click in the box and choose Paste).
5. Do not include your name or any other identifying information in the box.  You will give that information in steps 1-4, but it will not appear on your access codes page.
6. Do not delete or otherwise tamper with the SEND files in your c:\vhmthphy folder, which you might need for backup.

Spammers have caused some moderate problems with this form, and this must be dealt with manually.  Any manual process, no matter how carefully done, opens us to the possibility of clerical error.

For this, as well as to protect against possible software errors, it is recommended that you send a copy of your form confirmation using the Form Confirmation Backup

This is easily done.  After submitting your content, you will get a Form Confirmation.  Simply copy that content, return to this page (or alternatively have the form bookmarked), paste the content into the Form Confirmation Backup and submit.  We will keep your confirmation in our records.
