I am confused about what work is required for submission for a grade. I understand that labs are required, and the queries, but what else under assignments must be submitted through a form? Also, I would like more information on the test. What is the test like? What would be the best way to review for a test? Are we allowed a formula sheet or must we memorize all the equations?

You need only a few fundamental equations and definitions, and the ability (which we are working to further develop) to reason from those equations. So formula sheets are not permitted.
Having worked all the assigned problems you should be well prepared for the test. I recommend writing down the information you would most like to be able to take with you into the test were you to handwrite it on an index card (which per the above you're not allowed to take into the test). Then be sure you know that information when you go in.
