Proctor needs form completed

I sent you an email and wanted to also send the information through this form to let you know that the testing center at Mountain Empire Community College needs you to complete an instructor transmittal form before I can take my MTH 174 exams at the testing center. The form is attached as a word document to the email I sent you. After completion, you can email the form back to my proctor, Peggy Gibson, at the MECC testing center. Her email is pgibson@mecc.edu. Please let me know when you send this form to MECC so I can arrange to take my first test for the course. I am really enjoying this course and feel like I'm learning a lot!
