5.14.3 Procedures
to Request Access to Computing Resources page topThe following procedure is used by
college supervisors to request computer
access for their employees to the VHCC
Local Area Network (LAN), VHCC Email
System, VCCS Mainframe System, VCCS
Student Information System (SIS) and the
VCCS Administrative Information System (AIS).
There are two forms used in this
procedure:
- “Information Technology Employee
Ethics Agreement”
- “Request for Computer Access”
- Each new faculty or staff user must
complete the “Information Technology
Employee Ethics Agreement”. This form
may be obtained by the supervisor from
the DOCS folder on the Z: network drive.
By reading and signing this agreement,
the employee agrees to abide by all
applicable state, federal, VCCS, and
VHCC policies, procedures, and standards
that relate to the VCCS Information
Security Policy, the VCCS Computer
Ethics Guideline and Chapter 52 of the
Code of Virginia, which addresses
restrictions on state employee access to
information infrastructure. The VCCS
Information Technology Employee Ethics
Agreement should be returned by the
supervisor to the VHCC LAN
administrator.
- The supervisor must complete the
“Request for Computer Access
Form”. This form may be obtained from
the DOCS folder on the Z: network drive.
The supervisor completes the form and
either submits it to the LAN
administrator or forwards the form to
the appropriate Data Owner as
appropriate. The Data Owner signs the
form and forwards to the LAN
Administrator.
- Request for access to VCCS
Information Technology Services (ITS)
applications will be sent to the ITS
helpdesk by Library and Instructional
Technology (LIT) personnel to add or
change ITS applications access.
- The supervisor is responsible for
notifying the LAN administrator when an
employee is no longer eligible for VHCC
and VCCS ITS computing resources
applications or needs additional access
by re-submitting the “Request for
Computer Access” form.
- Once each year, the supervisor must
re-certify that current levels of
computing resources access is
appropriate for each or their employees
who have been granted access. The
supervisor will use the “Computer
Re-Certification Form 6.07.06” to
complete this requirement. This form may
be obtained by the supervisor from the
DOCS folder on the Z: network drive.
